His life turn upside down on sudden arrival of mysterious infant named Reborn. He was told the he was chosen to be the next boss of well prestigious mafia family, the Vongola. As seeing Tsuna have 0% capabilities of being a suited mafia boss, the current Boss (9th) sent Reborn to be his home tutor. He is currently taking harsh mob boss training program under the baby Mafioso, Reborn.
Although he is the tenth generation Vongola boss, he always refuses that he is a Mafia boss and tries to avoid anything that concerns the Mafia. In short, Tsuna doesn’t want to take part in anything that is related with the Mafia. But he wasn’t chosen randomly, in reality, his family is blood related with this mafia group. But Tsuna is in fact not the first choice (for obvious reasons). The most qualified Enrico was shot to death, Matsumoto the second one was drowned and the favorite child Frederico was reduced to bone. The only one left was Tsuna.
(That was until Xanxus was introduced and according to Basil’s story he was the one chosen by the other members. It was Kyudaime and Iemitsu who choose Tsuna over him.)
Tsuna is a descendant of the first boss and the founder of the Vongola Family. He is the great-great-great grandson of the first Vongola boss (Primo), Giotto. A person who has have the Vongola blood are the only ones who can ne its boss. There are lots of resemblances with Tsuna and his great grandfather. It’s been running in the series how they compare Tsuna to Giotto, for example his techniques, weapons, mannerism and others. Even his guardians are also compared.

Cervello to Xanxus; Chapter 135: Party
At the end of the Ring Conflict, Xanxus commented to Cervello that this is what they predicted (meaning they knew Tsuna will win). But Cervello said that they didn’t predicted nor their wish but it was something meant to happen. So Tsuna as the Vongola Boss is something already destined.
He still find the position too great for him but he slowly accepting it, in fact, he is grateful for the friends he had made since meeting Reborn. He cares a lot for his new 'family' and would put himself in the way of danger in order to protect them.
